An estimated 8.8 million hospital patients suffer from preventable medical harm each year (CR, May 2018, page 5). As many as 440,000 Americans die every year from medical errors and infections contracted in the hospital. Errors are often due to medication and diagnostic mistakes. Fast paced ER environments lead to a higher percentage of conditions being misdiagnosed or missed. Every year 700,000 to one million patients fall in the hospital and 30 to 50 percent end up with serious injuries such as broken bone or concussion with many of these injuries being preventable.
